If you searched F32.9 next to HCC 155 or V28, you are looking at one of the most common depression traps under CMS-HCC V28 for payment year 2026. F32.9 is billable major depressive disorder, single episode, unspecified. It does not map to a CMS-HCC V28 payment HCC.
The moderate codes that do map are F32.1 (single episode, moderate) and F33.1 (recurrent, moderate). Both map to V28 HCC 155 (Major Depression, Moderate or Severe, without Psychosis) with a source-labeled community non-dual aged (CNA) reference of 0.299. Severe without psychosis (F32.2 / F33.2) also maps to HCC 155. Severe with psychotic features (F32.3 / F33.3) maps to HCC 152 (CNA 0.484).

Coefficients are source-labeled CNA references from the live cards and the HCC 155 / HCC 152 hubs. Not member totals. Hierarchy still applies: HCC 155 is superseded by HCC 151, 152, 153, or 154 when a higher category in the same disease group is also present.
What F32.9 is (and is not)
F32.9 means major depressive disorder, single episode, unspecified. Inclusion terms include major depression NOS when the chart supports a single-episode MDD diagnosis without documented severity.
Billable does not mean risk-adjusted. Under PY2026 community V28 payment, F32.9 has no payment HCC. The same is true for F33.9 (recurrent, unspecified), mild codes (F32.0 / F33.0), remission codes (F32.4 / F32.5 / F33.40 / F33.41 / F33.42), and F32.A (depression, unspecified). Do not invent a V24 map for current-year non-PACE scoring. Select the historical period on the live card if you need older model context.
RxHCC may still show a Part D depression category for some of these codes. That is not a CMS-HCC V28 community payment map. Keep the models separate.
When severity and recurrence change the code
If the provider documents moderate severity on a first episode, use F32.1 (HCC 155). If the chart supports recurrent moderate MDD, use F33.1 (HCC 155). Do not leave a supported moderate chart on F32.9 or F33.9.
Pick the path from the documentation:
1. Single-episode MDD, severity not documented → F32.9 (no V28 payment map). Query when the note supports severity.
2. Single-episode MDD, mild → F32.0 (no V28 payment map).
3. Single-episode MDD, moderate → F32.1 → HCC 155 (CNA 0.299).
4. Single-episode MDD, severe without psychotic features → F32.2 → HCC 155.
5. Single-episode MDD, severe with psychotic features → F32.3 → HCC 152 (CNA 0.484).
6. Recurrent MDD with the same severity ladder → F33.0 / F33.1 / F33.2 / F33.3 (same V28 outcomes as above).
7. Remission codes → billable when supported, no V28 payment map for the F32.4 / F32.5 / F33.4x family in this desk.
Do not invent severity to create HCC 155. Query when PHQ-9, functional impairment, or the assessment narrative supports mild, moderate, or severe. Do not code single episode when prior episodes are documented (Excludes 1 on F32 points to F33.- for recurrent depressive disorder).
Mild, remission, and unspecified: billable, not V28 payment
Coders often assume "any MDD code" pulls HCC 155. Under V28 it does not.
Specificity still matters for the claim and the audit trail. It does not create a V28 payment HCC where the file has no row.
MEAT for HCC 155 / 152 capture
A mapped code still needs monitoring, evaluation, assessment, or treatment in the eligible encounter year. A problem-list row that says "depression" or "MDD" from last year does not carry HCC 155 or 152 forward by itself.
For F32.1 / F33.1 / F32.2 / F33.2 (HCC 155) and F32.3 / F33.3 (HCC 152), the note should support:

Audit and query traps
1. Leaving supported moderate MDD on F32.9 and expecting HCC 155 because "depression is an HCC."
2. Coding F33.9 or F32.A and assuming any depression label pays under V28.
3. Coding single episode (F32.1) when the chart shows prior episodes (should be F33.1).
4. Inventing moderate or severe severity from a vague "depression" note without provider confirmation.
5. Treating remission codes as active HCC 155 capture.
6. Confusing RxHCC depression mapping with CMS-HCC V28 community payment.
7. Skipping MEAT for a historical MDD that has no current assessment or treatment.
8. Ignoring hierarchy: when HCC 152 (or 151 / 153 / 154) also applies, HCC 155 can be superseded.
