Code changes and tool availability

FY2027 ICD-10-CM is available in Code Book and the encoder. Start with the date that applies to the record, then check the selected edition before choosing a code. Historical editions remain separate. Check the served edition and source status.

190 new billable codes; 30 previously billable codes stop being billable. The 30 consist of 15 removed codes and 15 retained entries that become headings. Added and removed entry totals also include nonbillable headings.

A date-first workflow for the October update

Keep the ICD-10-CM edition and the HCC model year separate. October 1 starts FY2027 ICD-10-CM; it does not turn payment year 2026 into payment year 2027.

Confirm the date that applies. Use the encounter date or inpatient discharge date as directed by the official guidelines. A September 30, 2026 record and an October 1 record can require different code choices.
Select that date in the tool. Check the edition shown before searching. FY2026 April covers April 1 through September 30, 2026. The registered FY2027 source covers October 1, 2026 through September 30, 2027, including April 2027.
Read the current description and billable status. A familiar code may now be a heading with child codes. Open the Tabular instructions and check the Index against the documentation.
Choose the model and payment or benefit year separately. Read the source status on the HCC card. A pending mapping is not a verified no-HCC result, and a billable code alone does not establish an HCC or RAF value.
Keep the date with the comparison. Use a historical edition for a historical record. Select a future date only when its source is available. If the source is unavailable, resolve that gap before relying on the result.

What each change means when you code

Added: The entry is present in FY2027 but absent from FY2026 April. The 238 added entries include 190 billable codes and 48 headings. Check the billable flag before selecting a new entry.
Removed: The entry is absent from the FY2027 order file. The 21 removals include 15 formerly billable codes and 6 headings. Search the new edition and review the documentation; a removed code does not establish a one-to-one replacement.
Expanded into child codes: A retained parent can become nonbillable while more specific children are added. Fifteen previously billable entries become headings in this comparison. Open the family and read each child description.
Description changed: Four retained entries have a different official description. Read the complete new wording even when the code number stays the same. Description changes and billable-status changes are separate checks.

A FY2027 expansion to practice

I42.0: a billable FY2026 code becomes a FY2027 heading

I42.0, Dilated cardiomyopathy, is billable in FY2026 April. For FY2027 it remains in the code set as a nonbillable heading. The new billable children are I42.00, Dilated cardiomyopathy, unspecified; I42.01, Familial-genetic dilated cardiomyopathy; and I42.09, Other dilated cardiomyopathy.

Practice: compare the same family for September 30 and October 1. On the October record, read the child descriptions and Tabular instructions against the provider documentation. The old parent code and its HCC result are not substitutes for checking the new child.

We compare code identifiers, exact descriptions and billable flags in the official order files. The separate FY2027 codes file independently agrees on 74,879 billable codes. The count reconciles from 74,719 in the April edition: add 190, subtract 15 removals and 15 changes to headings. No heading becomes billable in this comparison.

CMS also lists a separate PY2027 Python model software package, last modified September 28, 2026. Its V28 mapping contains none of the 238 new FY2027 entries. A new software download does not by itself establish a verified HCC mapping or coefficient for a new code.

When do the FY2027 ICD-10-CM codes take effect?

The registered FY2027 ICD-10-CM release covers October 1, 2026 through September 30, 2027. Use the edition that applies to the encounter or inpatient discharge date. A later April update needs its own published source files.

How many codes changed for FY2027?

The registered CMS order files contain 238 added entries (190 billable codes and 48 headers) and 21 removed entries. There are also 4 retained-description changes and 15 retained-code billability changes.

Which edition applies to a date of service?

Use the edition effective for the encounter or inpatient discharge date, as directed by the official guidelines. The April 2026 edition covers April 1 through September 30, 2026; the registered FY2027 annual edition covers October 1, 2026 through September 30, 2027. Choose an available edition for an explicit past or future date.

Do the new FY2027 codes map to HCCs for risk adjustment?

The registered PY2027 Initial mapping covers FY2025 and FY2026 codes and contains none of these FY2027 additions. Their mapping status is pending. Absence from that older code vintage does not establish that a new code has no HCC. Check the applicable payment-year source separately.

How current is this page?

The six registered source files were downloaded again on October 1, 2026 and matched their recorded checksums. FY2027 ICD-10-CM is available in Code Book and the encoder. HCC mappings and payment-year model sources must be checked separately.
