FY 2027 IPPS: CMS adds MS-DRGs to post-acute transfer and special payment lists (CR 14592 / R13930CP)

On September 11, 2026, the Centers for Medicare & Medicaid Services issued Transmittal 13930 / Change Request 14592 (Pub 100-04 Medicare Claims Processing) and the companion MLN Matters article MM14592, titled *Fiscal Year (FY) 2027 Inpatient Prospective Payment System (IPPS) and Long-Term Care Hospital (LTCH) PPS Changes*.

For inpatient coding and billing desks staging the October load, the post-acute transfer slice is the part to flag today: CMS is expanding which MS-DRGs sit on the post-acute care transfer policy list and the special payment policy list after reviewing FY 2027 MS-DRG changes against the criteria in 42 CFR 412.4(c).

What CMS added to the post-acute and special payment lists

MM14592 and Transmittal 13930 print the same evaluation result:

> We evaluated changes to MS-DRGs for FY 2027 against the general post-acute care transfer policy criteria using the FY 2025 Medicare Provider Analysis and Review data per regulations under 42 CFR 412.4(c).

> As a result, we're adding MS-DRGs 210, 211, 361, 362, 400, 403, 404, 456, 457, 458, 523, 524, and 525 to the list of MS-DRGs subject to the post-acute care policy. We're also adding MS-DRGs 361, 362, 400, 403, 404, 456, 457, 458, 463, 464, 465, 616, 617, and 618 to the list of MS-DRGs with special payment policies.

CMS tells readers to use Table 5 of the FY 2027 IPPS/LTCH PPS final rule for the full post-acute and special post-acute MS-DRG lists. Business requirement 14592.10 also directs the Common Working File (CWF) to update Informational Unsolicited Response (IUR) 7272 and 7800 as necessary for the post-acute DRGs listed in Table 5 when changes are made.

Dates that matter for the desk

Discharges on or after October 1, 2026, are the payment year these list adds apply to. Systems implementation is printed as October 5, 2026.

• Treat the MS-DRGs listed above as newly subject to the post-acute care transfer policy and/or special payment policy for FY 2027.
