New, invalid, and revised codes (Tables 6A through 6F)

6A, 6C, and 6E are diagnosis: new codes, invalid codes, revised titles. 6B, 6D, and 6F are the same three files for procedures.

These are the IPPS tables used for FY 2027 MS-DRG grouping. They do not replace the CDC/NCHS FY 2027 ICD-10-CM code changes. Use Table 6A when the question is how a new diagnosis sits in the FY 2027 grouper (CC flag, MDC, MS-DRG). Use the addenda when the question is whether the code exists in the classification.

After you have the table open, look the code up in the encoder if you need the current tabular note.

Table 6K's own header says it lists codes defined as a CC or MCC when used as a secondary diagnosis, and that a table number groups the principal diagnoses that make that CC or MCC count as a non-CC.

Table 6J.2 is the CC-deletion list. Load it when a diagnosis drops off the CC list for FY 2027 grouping. A separate note covers one CC-to-NonCC policy in this rule. That note is not this file list, and Table 6J.2 is not limited to that one policy.

If last year's CC and MCC flags are still on your cheat-sheet, replace them from 6I, 6J, 6I.1, 6J.1, and 6J.2 before the first October 1 discharge.

This is MS-DRG severity, not CMS-HCC V28

An IPPS CC or MCC flag decides whether a secondary diagnosis moves an inpatient stay into a with-CC or with-MCC MS-DRG. It does not assign a CMS-HCC, and it does not set a RAF.

For Medicare Advantage risk-adjustment work, use the CMS-HCC model for the payment year you are coding. Confirm the mapping in ICD-10 to HCC. Do not copy an IPPS CC flag onto a V28 recapture list.

The code book is still the classification. Table 6 tells you how FY 2027 IPPS treats the code as a CC, MCC, or NonCC.

  • Table 15: Hospital Readmissions Reduction Program payment adjustment factors. CMS will post it after hospitals review and correct their calculations, for discharges on or after October 1, 2026.
  • Table 16B: Actual Hospital VBP payment adjustment factors, available in the fall of 2026. Table 16A is posted and is proxy only. CMS says those proxies will not be used to adjust hospital payments.
  • MAC Implementation Files 1, 2, 5, and 9: will be posted at a later date.

Do not wait on Table 15 to grab Table 6. The code-and-severity packet is already up.

What coders should do now

  1. 1Download Tables 6A-6K (ZIP) from the FY 2027 IPPS Final Rule Home Page (fy2027-ipps-fr-tables-6a-6k.zip) and keep the Excel workbook plus the text files.
  2. 2Load Table 6A, 6C, and 6E for new, invalid, and revised diagnosis codes, and Table 6B, 6D, and 6F for the matching procedure files, before the first October 1, 2026 discharge.
  3. 3Replace last year's CC and MCC cheat-sheet from Table 6I, 6J, 6I.1, 6J.1, and 6J.2. Use Table 6J.2 when a diagnosis drops off the CC list.
