Key Takeaways
- →CMS issued Transmittal R13988CP (Change Request 14639, Pub. 100-04) on October 8, 2026: the January 2027 annual update of HCPCS codes used for Home Health consolidated billing enforcement. Effective January 1, 2027. Implementation January 4, 2027.
- →Effective for line item dates of service on or after January 1, 2027, contractors must remove HCPCS A4453 and A4459 from the HH consolidated billing non-routine supply code list (business requirement 14639.1).
- →A4453 (rectal catheter for a transanal irrigation system) was added to that same non-routine supply list only on October 1, 2026, under CR 14510 / MM14510, so it sits on the list for one quarter before this annual removal.
- →A4459 (reusable manual pump enema system) is also removed. It was not among the 19 codes added in the October 2026 quarterly update.
- →The HH consolidated billing therapy code list has no update in this CR. MLN Matters MM14639 was released October 9, 2026 for home health agencies and suppliers.
On October 8, 2026, CMS issued Transmittal R13988CP, Change Request 14639, the January 2027 annual update of Healthcare Common Procedure Coding System (HCPCS) codes used to enforce Home Health (HH) consolidated billing. It is effective January 1, 2027, by date of service, with an implementation date of January 4, 2027. MM14639 is the MLN Matters article; CMS released that article on October 9, 2026.
This is the Home Health PPS consolidated billing edit list, not the October 2026 SNF CB update. Skilled nursing facility consolidated billing is a different desk. It is also not an NCCI update.

Two non-routine supply codes come off the list
CR 14639 removes two HCPCS codes from the HH consolidated billing non-routine supply code list. Business requirement 14639.1 tells contractors to revise the edits so those codes are no longer enforced for claims with line item dates of service on or after January 1, 2027.
| Code | Long descriptor (from R13988CP) | Action |
|---|---|---|
| A4453 | Rectal catheter with or without balloon, for use with any type transanal irrigation system, each | Remove from HH CB non-routine supply list |
| A4459 | Manual Pump Enema System, Includes Balloon, Catheter And All Accessories, Reusable, Any Type | Remove from HH CB non-routine supply list |
MM14639 lists the same two code numbers and does not reprint the long descriptors. Use the CR when you need the wording.
While a code is on the HH CB non-routine supply list, Medicare will not pay it separately on dates when the patient is in an HH episode or under an HH plan of care that a home health agency administers. During an episode, Medicare reimburses only the primary home health agency that opened it. CMS lists three exceptions: therapies physicians perform, supplies incidental to physician services, and supplies used in institutional settings.
When A4453 and A4459 leave the list on January 1, 2027, those two codes no longer trigger that non-routine supply consolidated billing edit for later dates of service. Confirm the change in your MAC's HH CB files and your claim edits before you bill January dates.
A4453's one-quarter ride
A4453 is not a long-standing list member in this cycle. MM14510 / CR 14510 (Transmittal 13794, dated July 8, 2026) added 19 HCPCS codes to the same HH CB non-routine supply list effective October 1, 2026, with implementation October 5, 2026. A4453 is on that October add list, with the same rectal-catheter descriptor the January CR uses.
A4459 is not on the October 2026 add list. CR 14639 still removes it for January 1, 2027 dates of service. For historical include and exclude dates before these updates, use CMS's Home Health Consolidated Billing Master Code List on the HH coding and billing hub. For the October 2026 and January 2027 changes, use MM14510 / CR 14510 and MM14639 / CR 14639 instead.
So for A4453 the practical window is short: on the non-routine supply CB list for dates of service October 1, 2026 through December 31, 2026, then off the list for dates of service on or after January 1, 2027.
Therapy list: no update
CR 14639 also addresses the HH consolidated billing therapy code list. The entry is one line: No update.
Do not expect new therapy HCPCS on this annual package. If your HH therapy CB file needs a change, it will come in another CR, not this one.
How CMS frames these list updates
The CR background is the standing rule: CMS updates the HH CB code lists annually to match HCPCS code-set changes, and may issue quarterly updates when temporary HCPCS codes (for example, K codes) appear during the year. The new coding in each update "describes the same services that were used to determine the applicable HH PPS payment rates." CMS says no additional services are added by these updates; the lists move because the coding system changed, not because HH consolidated billing is being redefined.
Policy authority cited in the CR is section 1842(b)(6) of the Social Security Act, 42 CFR 409.100, and Medicare Claims Processing Manual, chapter 10, section 20.
What this is not
It is not the October 2026 SNF CB HCPCS update. That piece covers skilled nursing facility consolidated billing under a different CR. This CR is Home Health PPS consolidated billing.
It is not an NCCI procedure-to-procedure or medically unlikely edit update.
It is not a CMS-HCC, V28, or ICD-10-CM mapping change. A4453 and A4459 are HCPCS supply codes on an HH CB enforcement list. They do not assign an HCC. For diagnosis work on an HH episode, use the model year you are coding in ICD-10 to HCC and look up ICD-10-CM in the encoder or code book. For these HCPCS CB list changes, load the CMS CR, MM, and the HH coding hub files; do not rebuild the edit list from a blog dump.
What coders should do now
- 1Put a January 1, 2027 date-of-service note on A4453 and A4459: on or after that date they leave the HH CB non-routine supply enforcement list under CR 14639 / BR 14639.1.
- 2For dates of service October 1, 2026 through December 31, 2026, keep A4453 on your HH CB non-routine supply edit (CR 14510 added it). Do not drop A4453 early.
- 3Confirm A4459's remove-from-list action in your MAC HH CB file for January 2027 dates; it is in CR 14639 even though it was not on the October 2026 19-code add list.
- 4Leave the HH CB therapy list alone for this CR; R13988CP says no therapy update.
- 5Load the official files from the CMS HH coding hub and your MAC. For ICD-10-CM diagnosis work on the episode, use the encoder or code book; for these HCPCS CB codes, use the CMS CR and MM PDFs.
Frequently Asked Questions
What is CMS Change Request 14639?
CR 14639 is the January 2027 annual update of HCPCS codes used for Home Health consolidated billing enforcement, issued as Transmittal R13988CP on October 8, 2026. It is effective January 1, 2027, with an implementation date of January 4, 2027. MM14639 is the companion MLN Matters article released October 9, 2026.
Which HCPCS codes leave the Home Health CB non-routine supply list in January 2027?
A4453 (rectal catheter with or without balloon for a transanal irrigation system) and A4459 (reusable manual pump enema system). Contractors must remove both from the HH CB non-routine supply edits for line item dates of service on or after January 1, 2027.
Was A4453 just added in October 2026?
Yes. CR 14510 / MM14510 added 19 HCPCS codes to the HH CB non-routine supply list effective October 1, 2026, and A4453 is on that add list. CR 14639 then removes A4453 effective January 1, 2027, so A4453 is on the list for one quarter in this cycle.
Does CR 14639 change the Home Health CB therapy code list?
No. Transmittal R13988CP states that the HH consolidated billing therapy code list has no update in this annual package.
Is this the same as the October 2026 SNF consolidated billing update?
No. The October 2026 SNF CB piece covers skilled nursing facility consolidated billing under a different CR. CR 14639 is Home Health PPS consolidated billing for the non-routine supply list.
