HCC Category Mapping
RxHCCHCC 188 — Coronary Artery Disease and Angina
0.000What This Code Means
An abnormal bulging of a coronary artery wall that supplies blood to the heart muscle.
Coding Tips
- •Document whether the aneurysm is saccular or fusiform if specified
- •Identify which coronary artery is affected (left main, LAD, circumflex, RCA) for complete documentation
Clinical Significance
Coronary artery aneurysm is an abnormal dilation of a coronary artery segment, which can be congenital or acquired (from atherosclerosis, Kawasaki disease, vasculitis, or post-procedural). These aneurysms can predispose to thrombosis, embolization, and rarely rupture. While clinically significant and requiring ongoing surveillance, this code does not contribute to risk adjustment.
Documentation Requirements
- ✓Documentation of coronary artery aneurysm
- ✓Which coronary artery is affected (left main, left anterior descending, left circumflex, right coronary artery)
- ✓Type of aneurysm (saccular versus fusiform)
- ✓Size and extent of the aneurysm
- ✓Etiology if known (atherosclerotic, post-Kawasaki, iatrogenic)
- ✓Imaging confirmation (angiography, coronary computed tomography angiography)
Excludes 1 — Do NOT code together
- congenital coronary (artery) aneurysm (Q24.5)
Commonly Confused Codes
I25.3 — Aneurysm of heart: aneurysm of the heart wall, not the coronary vesselI25.42 — Coronary artery dissection: tearing of the artery wall, not aneurysmal dilationI25.10 — Atherosclerotic heart disease: may coexist but is a different conditionI71.x — Aortic aneurysm codes: different vessel entirelyI72.8 — Aneurysm of other specified arteries: for non-coronary peripheral arterial aneurysms
